What it most likely means is that you have improperly changed your VPN config (or mis-configured it in the first place) and you have a DNS leak. Warnings like this are usually a DNS redirect (e.g. you lookup thepiratebay.org and your ISP DNS sends you the address of nasty-warning.gov) and so you need to make sure your DNS is using the VPN. If you haven’t changed anything recently then it is possible that your ISP did something like turn on IPv6 and this is what is leaking. For the latter case, turn off IPv6 while using your VPN as a test.

If all else fails configure VPN to use port 443 so it looks like normal HTTPS traffic.
This is almost certainly caused by using your ISPs DNS instead of the VPNs. How are you connected to the VPN and have to manually changed DNS settings anywhere?
