1) Yes – I live out in the country (UK) and envy your speeds, but my VPN doesn’t have much of an impact.
2) I run the program on my phone and laptop. But it can be installed at the router, I just haven’t done this.

4) That depends on you, the UK government have decided they want ISPs to keep track of your internet traffic and have access to it without your knowledge, I don’t like that idea I don’t want my ISP to see my traffic. I’ve nothing to hide, but, to me, that’s akin to letting someone in my house to rummage around through my personal belongings. That’s totally your choice. Do you want your whole network using the same vpn? Or just certain devices.

Likewise, you could get a VPS (virtual private server) and do it yourself.

That is your decision. Do Virgin Fibre sell your surfing data? If you want your privacy from your ISP and if you want to use another network (to increase better network performance) from Virgin Fibre, it would be something to consider.
With the Facebook scandal we see more and more the need for privacy, since ‘you’ have become the product. All information about ‘you’ (like from Facebook and Google, from your smart phone) can and will be sold to the highest bidder, whoever that may be.
On cost: if you look around for deals and discount codes, you can get some amazingly low prices on lifetime subscriptions, even for a few fairly well-known VPNs. Such as $50 for a lifetime subscription.
Honestly I would recommend a VPN to most people. It’s just one of the many steps that’s needed to protect your privacy. However not all VPNs are created equal. Some providers can be malicious and do a number of things to you from injecting malware, to logging your activity, to selling your information to third party entities. Do your research and choose your provider accordingly. Some people may use alternative methods of tunneling, like SSH, but VPN is definitely probably the easiest and most common.

2.) Depends on the VPN. Some good routers have OpenVPN installed and if you install it on your router, all devices in your LAN will be routed through the VPN.
3.) Depends on the VPN. Expect about $100 / year for a good one.
4.) Yes. Otherwise your ISP knows every site you visit and you can bet your ass that your browsing records are being sold to someone. The good VPNs work with Netflix.
