Some might use VPSes but I’d imagine most VPN services that were serious about privacy would use physical servers.
Pretty much all of them host their services from proper datacenters though. They need some good bandwidth to satisfy everyone

This is a very good question. Likewise, you could ask where your ISP gets its own bandwidth from.
Regarding VPN services, most make use of data centres, as does any other internet service (like Facebook, Google, Yahoo, etc).
One example is Hetzner in Germany. You place your server(s) with them for a modest fee and you make your income from that server(s).
However, keep in mind that from the fact that most VPN services use data centres for their services, companies which don’t tolerate VPN’s (like Netflix, Hulu and others) may deny you service since you’re using a data centre IP, instead of a residential ISP ip-number.
We partner with IP Vanish. Their primary business is actually a CDN so they own all their own hardware (this is one of the primary reasons we chose them). One of our competitors runs on AWS so that’s the two extremes really. Everything in between happens.

I use ExpressVPN and thanks to their quick customer service email support I was able to confirm that at each location a physical server is present. Honestly, I don’t know how this relates to bandwidth, however, although I didn’t reply to dan4334, I think it relates some of his response.
